Terms and Conditions

1. Membership Enrollment and Renewal

All new enrollments are subject to a $25 enrollment fee. Memberships will automatically renew on the same date each month or year (depending on your plan) and will be billed to the payment method on file unless cancelled prior to the renewal date.

  • Monthly Plans: Billed automatically on the same day of the next month.
  • Annual Plans: Billed automatically on the same day of the following year.

2. Purchase and Renewal Conditions

By joining a plan, either for yourself or on behalf of a minor child for whom you are a parent or legal guardian, you confirm that you are at least 18 years old and you authorize American Dental Care Partners, Inc. to charge your credit card or checking account for the plan you have selected. This charge will automatically renew at the end of your membership term, and your credit card or checking account will be automatically charged for the appropriate amount, until you notify American Dental Care Partners, Inc. via phone or email that you wish to cancel the plan. By joining, you confirm that you have read and agree to these terms and conditions.

3. Cancellation Policy

Cancellations must be submitted via phone or by email to adc@americandentalcareinc.com.

  • Cancellations within 30 days of enrollment will receive a full refund.
  • After 30 days, monthly members agree to maintain membership for a minimum of 12 months. Cancellations before 12 months will incur a $50 early termination fee.
  • When you cancel, you will continue to have access to the plan for the remainder of the period for which you have paid. Your membership will terminate at the end of that period.

4. Termination Conditions

American Dental Care Partners, Inc. reserves the right to terminate plan members for any reason, including non-payment. If American Dental Care Partners, Inc. terminates the plan or your membership for any reason other than non-payment, you will receive a pro-rata refund of your membership fees.

5. Returned Checks and Late Fees

Returned checks will result in a $30 fee. A late fee of $4 per month will be assessed for each month that membership fees remain past due.

6. Florida Residents

Residents of Florida are not subject to the one-year membership commitment, late fees, or returned check fees.

Annual memberships in Florida are entitled to a pro-rata refund whenever you cancel.

7. Disclosures

By enrolling in this membership plan, you acknowledge and agree to the following disclosures:

  • This plan is NOT insurance.
  • The plan provides discounts at participating health care providers for medical services.
  • The plan does not make payments directly to providers for medical services.
  • Plan members are responsible for paying for all health care services but will receive a discount from participating providers.
  • Discounts apply only to those health care providers who have contracted with American Dental Care Partners, Inc.
  • American Dental Care Partners, Inc. does not guarantee the quality of the services or products offered by individual providers.
  • If you remain dissatisfied after completing our complaint resolution process, you may contact your state insurance department.
  • American Dental Care Partners, Inc. is a licensed discount plan organization with a principal address at 2617 Bissonnet St #440, Houston, TX 77005.

8. Savings and Provider Participation

Savings are based upon the providerā€™s normal fees. Actual savings may vary depending on location and specific services or products purchased. Please verify available discounts with each individual provider.

  • The planā€™s discounts may not be used in conjunction with any other discount plan or program.
  • All listed or quoted prices are current prices with participating providers and are subject to change without notice. Any procedures performed by a non-participating provider will not be discounted.
  • From time to time, certain providers may offer products or services to the general public at prices lower than the discounted prices available through this plan. In such cases, members may avail themselves of the lowest price.
  • Providers are subject to change without notice, and services may vary by state. It is the memberā€™s responsibility to verify provider participation in the plan.
  • American Dental Care Partners, Inc. cannot guarantee the continued participation of any provider. If a provider leaves the plan, members will need to select another participating provider.

9. Complaint Resolution

We are committed to addressing any concerns or complaints related to your membership or the services provided:

  • Our team will record your complaint and work with you to resolve the situation satisfactorily.
  • If you remain dissatisfied after completing our complaint system, you may contact your stateā€™s insurance department for further assistance.
