Terms and Conditions
Please read these terms and conditions carefully before enrolling in our dental discount plan.

1. Membership Enrollment and Renewal
There is a $25 enrollment fee that will be charged at the time of enrollment. This fee covers administrative costs and account setup.
Your membership will automatically renew for successive terms unless cancelled in accordance with the cancellation policy below. Members may choose monthly or annual billing cycles.
2. Purchase and Renewal Conditions
You must be at least 18 years old to enroll in this plan. By enrolling, you authorize American Dental Care to charge your chosen payment method for membership fees and any applicable charges.
By completing the enrollment process, you acknowledge that you have read, understood, and agree to be bound by these terms and conditions.
3. Cancellation Policy
You may cancel your membership by calling 713-784-9696 or emailing adc@americandentalcareinc.com.
If you cancel within 30 days of enrollment, you will receive a full refund of all fees paid.
After the 30-day period, there is a 12-month minimum commitment. If you cancel before completing 12 months of membership, you will be charged a $50 early termination fee.
4. Termination Conditions
American Dental Care reserves the right to terminate your membership at any time for violation of these terms. In case of termination by the company, you will receive a pro-rata refund for any unused portion of your membership fees.
5. Returned Checks and Late Fees
A $30 fee will be charged for any returned checks or failed payment methods.
A $4 monthly late fee will be applied to accounts with overdue payments.
6. Florida Residents (Special Conditions)
Florida residents are not subject to the one-year minimum commitment requirement.
Florida residents will not be charged late fees or returned check fees.
Florida residents have the right to a pro-rata refund upon cancellation at any time.
7. Important Disclosures
THIS IS NOT INSURANCE. This is a discount plan that provides access to discounted dental services from participating providers.
Members are responsible for paying all dental costs directly to the provider at the time of service, minus any applicable discounts.
American Dental Care does not guarantee the quality of services provided by participating dentists. Members should verify provider credentials independently.

American Dental Care Partners, Inc. is licensed to operate in multiple states. License information is available upon request.
8. Savings and Provider Participation
Savings amounts may vary by provider and geographic location. Actual savings depend on services received and provider participation.
Provider participation may change without notice. We will make reasonable efforts to maintain an adequate network of providers.
Members will be notified of significant changes to pricing or benefits with at least 30 days advance notice.
9. Complaint Resolution
If you have a complaint, please contact us first at adc@americandentalcareinc.com or 713-784-9696. We will make every effort to resolve your concern promptly.
If your complaint is not resolved to your satisfaction, you may contact your state's insurance department or regulatory agency.
